This Notice of Privacy Practices (“Notice”) describes how your medical information may be used and disclosed by
BeMediq-affiliated providers, including
Beluga Health, P.A. and similar affiliated professional entities (“Providers”), and how you can access this information. Please review it carefully.
Our Responsibilities
By law, your Providers must:Maintain the privacy of your
Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state laws.Provide you with this Notice describing legal duties and privacy practices.Notify you if a breach compromises the privacy or security of your PHI.Follow the terms of this Notice unless you authorize a change in writing.
How We May Use and Share Your Information
We may use and disclose your PHI for the following purposes
without additional authorization:
1. Treatment
To provide, coordinate, or manage your healthcare. For example, your Provider may share PHI with another healthcare professional or pharmacy to ensure appropriate care and prescription fulfillment.
2. Payment
To bill and collect payment for your care. For example, we may share your PHI with your health plan (if applicable) or with our partner pharmacies.
3. Healthcare Operations
For internal operations such as quality improvement, training, and audits to ensure you receive quality care.
4. Required by Law
We may disclose your PHI when required by federal, state, or local law, including:Public health reporting, such as reporting communicable diseases.Suspected abuse, neglect, or domestic violence.To prevent or reduce a serious threat to anyone’s health or safety.In response to a valid court order, subpoena, or other legal process.
5. Other Permitted Uses
We may use de-identified or aggregated information for research, education, or analytics, provided it does not identify you.
When We Need Your Authorization
We will
never sell your PHI or use it for marketing without your written authorization. Any uses or disclosures not described in this Notice require your explicit permission. You may revoke any authorization at any time by contacting us in writing.
Your Rights Regarding Your Information
You have the following rights regarding your PHI:
Get a Copy of Your Medical RecordsYou can request to see or get an electronic or paper copy of your records.
Ask for CorrectionsIf you think information is incorrect or incomplete, you may request an amendment.
Request Confidential CommunicationsYou can ask us to contact you in a specific way (for example, by email or phone).
Request RestrictionsYou can request restrictions on how we use or share your PHI, although we are not required to agree in all cases.
Get a List of DisclosuresYou can request a list of certain disclosures of your PHI made in the last six years.
Receive a Paper Copy of This NoticeYou can ask for a printed copy at any time.
Right to File a ComplaintIf you believe your privacy rights have been violated, you may file a complaint:

BeMediq/Beluga by contacting us (see below), or
- With the U.S. Department of Health and Human Services, Office for Civil Rights.
We will not retaliate for filing a complaint.

Changes to This Notice
We may change this Notice and will post the updated version on our website at
https://www.bemediq.com. The updated Notice will apply to all PHI we maintain, including PHI we received before the changes.
Use of Technology Platforms
BeMediq provides the technology platform for telehealth services. All medical care is delivered by independent licensed providers, such as
Beluga Health, P.A.. BeMediq itself is
not a healthcare provider but follows strict HIPAA requirements when transmitting or storing PHI on behalf of Providers.
